Draining Huge Back Cyst
Draining Huge Back Cyst
Video Transcript
A clinic-room recording of a large back cyst being drained and removed. The audio is almost entirely the working conversation of the room — the team numbing the area with cold spray and lidocaine, joking with the patient, who goes by Cobra, and checking that he is comfortable as the cyst is opened and emptied. It plays as an unscripted procedure recording rather than a narrated lesson.

I'm the one who knows everything not him. You're the one who thinks you know everything. Are we going? Yes. We're five. We're five. That was kind of mean thing to say. All right, we've got our patient, Cobra. Cobra, thanks for letting us record your procedure for YouTube and the Internet in general. I'm not even going to do this. Are you with us, Cobra? Yep. You can talk. We're just getting ready to go here. Are you ready for this? I really would do it to it. I like that.
All right, first thing we're going to do is we're going to give you a little numbing medicine here. I'm going to get the gonds to spray some cooling or cold spray on you. And then we're going to start with an injection of some lidocaine. Are you ready? Yep. Here we go. Ooh, a little chilly, a little stick. What's it going to be, feel, Gonzo? Sticks, string, and a burn. Did we call it? He's focused. He's gone to his happy place. He's gone to his Cobra happy place, right?
Brave, Mr. Cobra, brave. This is the easy injection here. Got a good view, Gonzo? I sure do. He's squeezing like he's giving birth. Are you having a baby? He's having a baby. We've got Misty here. Here you go. I'm not counting. I have a feeling that's going to squirt. You think that's going to squirt? Yeah. So I'm going to hide behind Misty. All right, here's the next part of the numbing. Are you ready? Whatever. I'm going to go more in deep. This goes deeper. Deep breath. You're doing great, bud.
Inhale and exhale. Spray a little bit. Can you expect that? Oh, did you just bubble out just a little bit, Gonzo? Misty, how have you been? Fantastic. You were not included in some of the fun yesterday. Gonzo wasn't even here. I know. But we had Athena. We had a procedure yesterday. How'd that go, buddy? Not bad. Not too bad? So let's give it just a moment. Now, what kind of helicopters did you fly, Mr. Cobra? Cobra is Huey's. 46 is 53's. I'm not sure what 53 is.
Jolly Green Giant. Oh, really? That had the two giant rotors, right? No, the two is the 46. That's the 46, okay. 53 is basically a little bit bigger. Single rotor. How long have you had the cyst on your back here? Over two years. All right, we're ready to make our first incision. I believe. Are you ready? Never. Gonzo, you got a good view here? Yes, I do. You already have some pus come out here. Misty's ready for you whenever she needs her to go. We're going to do a culture here just in a moment.
We have our little volcano. What's making that in my head? Let me have the culture at please, ma'am. Let's get it done in there. I'm surprised you didn't let you do it. You couldn't do it. You handed it all. Well, you asked for it. I figured if you were going to do it, you'd have jumped right in. I don't let him do it. I do it. Okay, so we've got a lot of pus coming out here. Let's give it a squeeze. Can you see it okay? Yes.
Deep breath, buddy. Let's breathe. A lot of pus? A lot of pus. Move that box there, thank you. A lot of pus and a lot of cyst material coming out too. You're blocking. I'll keep working that out for you. Your brain just came out through your back. Sorry. Don't listen to her. We've got to have a brain first, though. See, I've been listening to women too much. I lost all my brain power. Did you really? Your wife is going to see this. She knows it. She keeps telling me she's the boss.
She'd be right about that. She's never wrong even when she is. That's right. Even when she is. That's good. I like that one. Now, because this is infected, Mr. Cobra, we're going to have to pack it open. You follow me? Yeah. We can't close this up with sutures or stitches because if we do, this abscess is going to want to reform. The problem is, is you've got a lot of cyst material in here along with infection. So some of this is infection. Some of it's just old cyst kind of mucinous material,
which is protein and oils and so forth that have kind of collected under the skin for some time. So all we're doing right now is just making sure that everything is opened up and draining well. Cobra, how are you feeling, man? Still here. Yeah, you are. Still here, aren't you? All right. I just sent them for some more numbing medicine because we're going to have to dig a little bit deeper in there, okay? Mm-hmm. All right, so here's what we're going to do, Ms. Cobra. We're going to give you another little injection
and we're going to be a little bit more aggressive trying to remove any tissue that's in there. And then after that, we're going to pack it and let you go. Okay. And you come back tomorrow around the same time or if you need to come in earlier. The infection doesn't help too much with the numbing, does it? Nope. Or you need more medicine sometimes for it. It causes a situation where you might need more medicine. That's exactly right. All right, let's get in there a little bit deeper now.
Right in there. That's what I'm looking for right there. Get close if you want. Ow! That was my knee. A little bit? Yeah. If we don't get that stuff out of there, then there's more of a possibility that this cyst will return later. So we've got to try to get the, make sure we get all the sac. The good thing was I think a lot of it came out. He did have a lot of pus come out for sure. He did, but he also had a lot of cyst material,
sac and otherwise. Yeah. There you go. They were called pork's head. Yeah. All right, so I went and I got the currette that was so generously donated to us by YellowjacketSoup. So we're going to use a currette to kind of scrape out this sac material that's in here, reducing your risk of this cyst actually returning down the road. I'm just going to let you know what we're doing. We're going to move over so that you kind of have an idea. Since you're awake anyway, right? Yeah. So when was the last time you flew a helicopter?
1978. It's been a little while, huh? Yep. It's been a minute, as Gonzo might say, right Gonzo? Y'all? Where's Misty going? She's going to have to go. I have to do a job. No, no. She has another job? She's got a good job. I do. Thank you though. You're welcome. So this is really going to help get some of this material out of here that we find undesirable. And you can see we've got the cyst all out of there and we've got a nice clean wound. Okay?
Yeah. And you can see we've got the cyst all out of there and we've got a nice clean wound. Okay? Good work. You go right ahead. The Gonzo's administering the dilucoroxides. What are you down in there? Good work. Let it bubble up. How you doing now, Cobra? You doing okay? Still hanging in. Still hanging in with us. All right, now you've got the Gonzo. She's going to pack it for you. I need another acute spacer. Sure. There you are, Dr. Gonzo. She's packing with the iota form gauze.
Good. Get it all in there. Okay. I've got a big space that was occupied by that infected cyst. The wound will be really deep. It is. It's real. I can't talk today. It's okay. You're doing a good job. She's almost done, Cobra. I'm actually much nicer than Dr. Gilmore on this. She really is. Not. Poke some inferiority, too. Don't just go up. Go down into the sides, too. There you go. There you go. Nice and slow. All right, while she's finishing that up, Cobra, I wrote you a prescription for some pain pills.
You've still got antibiotics, right? Yeah. You've taken those. I wrote you a prescription for some Zofran in case you get some nausea. Now, you may not want to get that filled immediately, but you can if you want to. Some people, when they take the pain medicine, it makes them feel nauseous, so you might need it. All right, she's going to put a dry dressing over this. We're going to ask you just to leave that in place until we see you tomorrow. Action. Stay tuned for the Meta 7 video.
I don't have to record you, but I really liked you. You're so photogenic and very happy. People want to know more about coconut anyway, so tell me what you were going to tell me. Coconut loves Meta 7, okay? Why? Because, look, I could not get comfortable for anything last night. I went to sleep at 1 o'clock this morning, woke myself up around 5, and I have been knocking patients out all day nonstop, and I still have a whole mess of energy. Thank you, Dr. Gilmore, for making Meta 7.
Okay, so you took Meta 7 this morning? Yeah, I take it every morning. You take it every morning. So even though you had a hard time sleeping last night, you took Meta 7 this morning, and now you are a medical assistant machine. I am a machine. I'm a tank.
