A lot of women come to us, asking about breast enhancement procedures. They come asking for options, about what we can do to increase the size of their breasts, the different options for the service, what are the options most commonly used in breast implant, and are there different types of breast implants available in the market?

There are two ways of increasing the size of the breasts. The first is fat grafting surgery & second is breast implant surgery.

Fat grafting surgery a procedure in which we remove the fat from certain areas of the body where the woman wants to lose fat, and this is fat creates a pad and is grafted to the breast to enhance the size of the breast. This is an excellent technique since you get an autologous tissue to augment their own size of the breast. But the problem with this technique is that you can only increase the size of the breast from half to 1 cup size. Now if a woman desires for the increase in the breast's size from A cup to a C or D cup, then the best option available for them is breast implants.

Now, breast implant surgery is one of the most commonly done surgeries for women across the globe. It's a safe surgery if done properly by an excellent surgeon at a good center. If you are considering breast augmentation surgery, this video is for you because we will discuss a lot of finer nuances in breast augmentation surgeries–especially implant-based surgeries.

So let's start with approaches of breast implant surgery:
There are essentially three approaches in which we can place a breast implant in any female.
Either you can give a trans axillary cut, in which the incision hides in the armpit, you can give a perioral incision, in which the incision is at the border of an areola, or an inframammary incident in which the incision is beneath the breast.

Now, let's know what these approaches are. This is an armpit approach in which we hide the incision or the scar in the axilla. This is a perioral approach, in which the incision is at the edge of the areolar and the normal skin, and this is an inframammary incision, in which we keep the incision beneath the breast. All these options have their own pros and cons.
What I recommend to every patient is to discuss each one of these options with their respective doctors in much more detail and then decide about the best approach or a plan for every individual.

In my practice, the most commonly used infra-mammary incision because this incision is always hidden under the breast and is not visible even if a patient is standing naked. This incision is a much more direct approach to give you a breast implant.

The other approaches also have their own advantages in certain indications.

How do you decide the size of an implant?
Traditionally, every surgeon was using a technique of putting the original sizes of implants inside the woman's bra and asking them to look in the mirror & see and decide what implants they would prefer; what size they would prefer. This has been the technique that has been done everywhere across the globe, but it has its own fallacy because the implant is not going beneath the screen. It is on the skin, under the bra to give you an idea. But this is not very accurate. The surgeon would always assess you for your shoulder weight, for your pelvic weight & will discuss with you what your desired goals are & then decide about the size of the implant.

Now, what are the effects of these techniques on your overall aesthetic result?

Also, it is recommended that one should do this technique if the tissue thickness in the upper pole of a female is less than 2 centimeters because if you put a breast implant in a thin female just behind the breast tissue and the tissue is less, the implant will show & it will look unnatural over the long-term.

The most important concern of the people who are considering this surgery is about recovery. Tummy Tuck Surgery or Abdominoplasty needs 2-3 hours for the surgery depending upon the extent of your fat. It is preferred usually to do a 360 liposuction.

Abdominoplasty or Tummy Tuck Surgery Recovery Time -
After the surgery, the patient stays 1 night or sometimes 2 nights in the hospital, but they start walking the same way. For the first 10-12 days walk with a slight bend so that your scar near to your bikini doesn't stretch. From the very next day, you can do your basic things at home like walking around, having food on your own, etc.
First 3-4 days there is a little pain and tightness in the patient. After 3-4 days, they start recovering well and the discomfort is much lesser. Every individual recovers differently as their pain threshold is different. Everyone has a different strength.

You can resume your job in 7-10 days. However, if your job has more physical work then they can resume in 15 days. You can resume your cardio exercises like running, jogging after 3-4 weeks. This is the basic timeline for recovery.

Antibiotics are given for the first 7 days. The dressing is removed after 3 days. After 7 days you are allowed to apply scar creams so that the scar becomes light enough.

The size of implant is chosen upon what are the patients expectations or requirements and also patients body shape.
We also perform 3D simulation preoperatively, where in patients body images are converted into 3D images and the patient can see how different size of implant will look on them . Depend on all these factors surgery is planned.
This surgery is a day care surgery . The patient can resume her work after the surgery .
Only precaution that patient needs to take is , she should not do weight lifting exercises for 6 weeks.

In Today's video, we will answer: Asian blepharoplasty or double eyelid surgery can be done for caucasian?

What is double eyelid surgery?
The people of an oriental race or south-east Asians have big upper eyelids or sagging upper eyelids which give a visual impression of having small eyelids. They do not form eyelid fold. A person who does not have an eye fold but desires to get an eye fold may have to undergo double eyelid surgery.

What is the upper eye fold?
When we observe the anatomy of upper eyelids there is a muscle known as levator palpebrae superiors (LPS) Which helps in raising and closing of the eyelids. Eyelid fold is formed only when we open our eyes this is because a thin layer of LPS muscle gets inserted into the skin. When this muscle contracts to open the eyelid, the layer of the skin where the LPS muscle attaches gets pulled in, which forms the eyelid fold.
The people who do not have upper eyelid fold means that they do have LPS muscle but this does not get inserted into the skin. So in Asian blepharoplasty or double eyelid surgery doctors insert this LPS muscle into the skin through a surgical procedure. Also, doctors access whether the patient has excess upper eyelid fat/ skin so they may remove it but the LPS muscle is sutured into the skin so that the upper eyelid fold is formed.

Asian blepharoplasty involves three things
1. Suturing/ stitching of LPS muscle to the skin
2. Removal of excess fat if needed
3. Removal of excess skin if needed

So now that we know what is double eyelid surgery lets answer some of the common questions
1. Asian blepharoplasty or double eyelid surgery can be performed on Caucasians?
The answer to this question is NO, as they do not require it because they already have an upper eye fold.

2. Can Double eyelid surgery be performed on white skin??
Yes. It can be performed on any colour of skin. It can be performed on anyone who desires to have an upper eye fold who does not have an eye fold. It is not related to any skin colour, it's just about whether the person has upper eyelid fold or not.

This is a daycare surgery. The surgery is done under local anaesthesia. The patient is discharged immediately after surgery. There will be swelling and bruising around eyes which settles down in a week. The results can be seen immediately.

I am Dr. Rajat Gupta, a board-certified plastic surgeon working in Delhi, with an experience of over 2000 body contouring procedures.
So, let’s talk about what is massive weight loss.
Now, any individual can lose a lot of weight. But massive weight loss means the loss of at least 10kg in 3 months time. So, lot of individuals lose a lot of weight either due to exercise, dieting or they undergo some procedures like bariatric surgery, in which there are different options of sleeve gastrectomy, roux en y bypass – all as simple as a balloon insertion into the stomach.
After all these interventions, a person loses a lot of weight and they reduce their subcutaneous fat a lot. Now, once they lose this weight and the subcutaneous fat what happens is that their skin starts sagging or the skin gets loose.
You may notice that they lose fat from their face and neck and the face and neck skin start loosening up. Or, majorly, they lose a lot of fat from their abdomen and their abdominal skin gets loose and it doesn’t look nice if they remove their clothes. There is a huge amount of skin which is sagging. At the same time,
they may have some loose skin in their thigh area which does not look nice.
In females, once they lose a lot of fat, they may also get breast sagging. Their breasts may sag. Which also doesn’t look nice. Depending upon what areas an individual has got sagging and in, we plan the correction for those areas. Basically, what they need is removal of excess skin.
Now, whether this excess skin is in their neck, in their chest, in their abdomen, in their arms or thighs, the treatment is customized depending upon what an individual’s needs or requirements are. So, suppose someone has a lot of excess skin in their abdomen. What it needs is the removal of excess skin from the lower abdomen or a procedure known as tummy tuck or abdominoplasty. If a person has excess skin in their thighs or arms, then they need a reduction in their excess skin, in those areas and the procedure is called a thigh lift or an arm lift. For face and neck sagging, a person may undergo a procedure
called face or neck lift. And for the sagging of breasts, they need a procedure called breast lift or mastopexy.
Now, all these procedures are actually customized and tailored for each individual. But a person who has lost a large amount of weight and has sagging skin, they would eventually need this because sagging skin is
not only an aesthetic concern but that area is also very difficult to clean. The sweat gets accumulated, they may have some fungal infections there, and it doesn’t look nice altogether. I’ve seen such individuals, when they remove their clothes, it’s not only that they don’t look nice, they also lose a lot of their confidence.
They display inhibitions even though they have lost such a large amount of weight and they look nice with clothes on. They would be inhibited by removing their clothes and going swimming or for a beach holiday.
So, understand that the ramifications of this procedure are not only for aesthetic concerns but also for the self-confidence of that person.
These procedures are reasonably safe procedures if done by a good plastic surgeon at a good center. Now, you need to choose your plastic surgeon keeping in mind a person who has experience with post massive weight loss contouring because these are some things which are very different to contour, the skin excess is in all dimensions, and therefore your plastic surgeon has to be smart enough and artistic enough that they understand how to remove the excess skin while giving you minimal scars and, at the same time,
enhancing the shape of your body.
