A manufacturer's technique presentation (CooperSurgical) for vacuum-assisted delivery with the Mystic II ventouse. It sets out safe technique for occiput-anterior and occiput-posterior deliveries and argues for matching the cup and tool to each birth rather than a one-size-fits-all approach, drawing the parallel with forceps delivery.

Cooper Surgical Presents Mystic 2, the safer, softer approach to vacuum-assisted delivery. Safe techniques for oxoput anterior and oxoput posterior delivery. For over 25 years, the mighty vac family of vacuum-assisted products has helped physicians deliver babies when operative vaginal delivery interventions are necessary. Similar to the art of forcep delivery, today's vacuum devices should not be a one-size-fits-all approach, since every delivery is unique. Having the most appropriate tool for each birth is important. The patented handle, coupled with specially designed cups, ensures Mystic 2 provides the clinician with the safest device for every operative vaginal delivery.
During an oxoput anterior delivery, the use of a mighty soft bell cup ensures the softest, most stable cup on the market is being utilized. The key features of this cup with the Mystic 2 handle are tissue-friendly soft rubber interface around the rim of the mighty soft bell cup. Freely rotating stem eliminates any possibility of placing torque during delivery. Along with an easy-to-reach release mechanism that allows you to push or pull to release vacuum pressure. A vacuum release port that allows you to confirm that air is eliminated from the chamber.
And most importantly, the Mystic 2 built-in safety valve that ensures the unit will not exceed the recommended 58 cm of mercury. Mushroom-style cups have been shown to have abrasion laceration rates as high as 14.1%. Therefore we suggest you use our mighty soft bell shaped cup for oxoput anterior deliveries. As per up-to-date, use of the mushroom-style cup is recommended for cases where more traction or flexibility may be required, such as an oxoput posterior presentation. During this OA delivery, the mighty soft bell is placed over the flexion point and delivery is accomplished within the next 2-3 contractions.
When more traction or an increased amount of flexibility is required, the semi-rigid M-style mushroom cup is suggested. This cup utilizes the same M-style design that has been in use for over 25 years as part of the mighty vac family. The semi-rigid material optimizes M-cup traction force while maintaining a scalp-friendly interface. The newest feature is the flexible joint that connects the stem to the cup. During deliveries that require a higher traction force, such as OP deliveries, an M-style mushroom cup selection is appropriate. These deliveries often require a flexible stem-cup interface to reach the media inflection point.
Therefore, this new cup's ability to be inserted smoothly makes it the right choice for all non-OA presentations. To summarize, as with the art of forcepuse, every delivery has different requirements to affect a safe outcome for the mother and the baby. A one-cup approach will not provide you with an optimum result for all your deliveries. By selecting Mystic II for your vacuum-assisted delivery product, you get unique features and two-cup variations for a softer, safer, operative delivery. The patented handle design not only incorporates an ergonomic grip, but a readily accessible vacuum release button and a built-in safety valve.
We recommend having a supply of both the Bell Cup for OxyPUT anterior presentations and the M-style mushroom cup for OxyPUT posterior presentations on hand to facilitate all deliveries.
