Key Steps — Stand-pivot transfer — bed to chair

A University of Nebraska Medical Center (CAPTURE Falls) staff-training video. The checklist follows the transfer as demonstrated.

  1. Assess the patient first: a stand pivot needs a patient who can bear weight through their legs and follow instructions. Get a second staff member if the assessment says so.
  2. Position the chair at a slight angle beside the bed, on the patient's stronger side; lock the brakes of both the bed and the chair, and swing away or remove the footrests.
  3. Sit the patient on the edge of the bed, feet flat on the floor a little apart, wearing non-slip footwear.
  4. Fit a gait belt if that is your local practice. Stand close in front of the patient, knees bent, bracing their weaker leg with your own leg if needed.
  5. Have the patient lean forward — nose over toes — with hands on the bed or the chair arms, never around your neck.
  6. On a count of three, the patient pushes up through their legs and arms while you guide and steady them. The patient does the work; you do not lift.
  7. Once standing, pause: let the patient find their balance and check they are not dizzy before moving.
  8. Pivot together in small steps toward the chair until the backs of the patient's legs touch the chair's edge.
  9. The patient reaches back for the chair arms, leans slightly forward, and lowers into the chair while you steady them.
  10. Settle the patient with feet supported and the call bell in reach, and document the transfer and how it was tolerated.
