the Pseotomy Repair. This patient has a partial third degree laceration. Begin by closing the fascial defect in the rectal sphincter. Identify and suture the apex of the vaginal defect. Use a running lock stitch to close the vaginal mucosa side to side. Continue the running lock stitch up to the hymenial ring. Bring the suture underneath the mucosa and close the defect between the bubocabrinosis muscles using a simple running stitch. Continue to work your way down towards the rectum closing the bubocabrinosis muscles from side to side. Bring the suture out through the inferior apex of the incision.

Then close the skin using a subcuticular stitch. Tie the suture when you have returned to the hymenial ring. Inspect your repair, then expel any clots from the vagina. Perform a rectal exam to ensure no sutures were placed through the rectal mucosa.
