The typical radiograph is of a well-defined, rounded, retrocardiac opacity with an air-fluid level. In this image, the radiolucent gas is highlighted in blue, while the gastric contents are highlighted in the green. In many cases of hiatal hernia, there will not be an air bubble below the left hemidiaphragm. This is a relatively expected finding considering that the stomach is no longer in its usual position. The anatomical position of the herniated organ can be further elucidated on the lateral radiograph. Here we can see that the stomach is in the middle mediastinum posterior to the heart and above the diaphragm. Hiatal hernias can look similar to a retrocardiac lung abscess or another cavitary lesion, but it will change in size and shape between radiographs. Large hernias can shift the mediastinum to the right and result in a widening of the carinal angle. They can even give the appearance of cardiomegaly. In this radiograph, the cardiac silhouette is distinctly visible within the confines of the hiatal hernia. To review, a hiatal hernia on an AP chest radiograph typically appears as a round retrocardiac opacity with an air-fluid level.

Thought a snake in your boot was bad? That old 19th-century idiom is nothing compared to one in your ear.

Shocking footage captured the alleged moment that a “surgeon” tried to remove a live snake that infiltrated a woman’s ear. Video of the herpetological surgery has racked up more than 125,000 views as viewers speculate whether or not the squirm-inducing footage is authentic.

“The snake has gone in the ear,” reads the caption to the bizarre Facebook clip, which was posted Sept. 1 by an India-based social media star named Chandan Singh to his 20,126 followers. However, it’s unclear where, when or how this unfortunate event transpired, local outlet the Economic Times reported.

In the nearly four-minute clip, an alleged medical practitioner can be seen using tweezers in a desperate attempt to extract a black and yellow serpent that’s peeking its head out from a female patient’s ear.

Today, the most common approach for open-heart surgery is a sternotomy, which requires a 12-14-inch incision through the breastbone. Vatche, Minassian, MD, MPH, Chief of Urogynecology, and Sarah Cohen, MD, MPH, Director of the Minimally Invasive Gynecologic Surgery Fellowship Program at Brigham and Women’s Hospital, perform a laparoscopic burch colposuspension, a procedure used to correct stress urinary incontinence.

Stress urinary incontinence is one of the most common types of incontinence and is characterized by urinary leakage during physical activities including coughing, sneezing, exercising, lifting, and laughing. As the condition progresses, it can become severe enough to happen with simple acts such as bending and walking. This condition is due to an anatomic weakness of the bladder neck which typically maintains the seal of urine during activity. Stress incontinence can result from a variety of conditions including vaginal childbirth, aging, menopause and obesity. As this is an anatomic condition, primary treatment may involve pelvic floor exercises and/or minimally invasive surgery.
